Before you may attend tryouts for any team, these items are REQUIRED to be on file with the school:

  1. Current Physical Form dated after May 1 of the current year. See below for more information.
  2. Concussion training attended by both parent and student. See below for more information.

Form must be handed in before tryouts begin!

All schools have a sports physical day. You may go to your family doctor or go to any school to obtain your physical. Date is TBD.

Important:
Parents - be sure to complete pages 1, 2 & 4 and sign all signature spaces on the form - including the last page! Many students miss the first days of team tryouts due to missing signatures.

Physicals are valid from May 1 of the current year through June 30 of the succeeding year and are required to be completed annually. (Must be dated May 1 or after for the 2019-2020 school year).

Athletic Physical Day at Unity Braxton Middle School

We will be offering physicals in the main gym one day only: Date is TBDThe cost is $30 - cash or check. Checks are made payable to Family Medicine. You may also complete your physical at your own doctor, other school physical sessions, or at most walk-in clinics but you must use the correct PWCS form and must be dated after May 1, 2020 for the 2020-21 school year. Students that do not have a completed form are not permitted to attend tryouts.

Due to COVID-19 the concussion training will be available online for all 6-12 grade students, starting July 10.

Q: Who needs training? 
A:   All student athletes and their parent/guardian need annual trainingView Policy 759

Q: I have a middle school student AND a high school student, which training do I need?
A:   Both

As there are sufficient differences in the way concussions will be handled at the middle school and high school levels, a parent having children in both middle and high schools is required to attend both a middle AND a high school presentation.

Training

Prince William County Public Schools is committed to providing face-to-face concussion training as the desired method of training for parents and student athletes. Occasionally, students and parents new to Prince William County will have registered or moved into the Division after the publicized face-to-face training or season and tryouts. In these instances, the parent may request access to a specific online training in order to meet the concussion training requirement and enable student participation in the desired activity. However, due to COVID-19 the concussion training requirement will be met by taking the online concussion training starting July 10. We will make every effort to accommodate for face-to-face concussion training, as needed.
